WHO IS ELIGIBLE FOR BENEFITS?

New Participants: Must be credited with a minimum of 160 hours within a 1-2 month period and be actively working on the first day of the month following the prior consecutive month(s).

Maintaining Your Coverage: Once your coverage begins, it remains in effect for as long as you continue to be credited with a minimum of 120 hours and are a full-time employee.

When Coverage Ends: Your coverage will continue to the end of the month following the month in which your employer stops contributing on your behalf. {i.e., Laid off, Leave of Absence, Military, or Retirement.}
**A Participant who voluntarily quits or is fired will have their benefits terminated a the end of the month in which they cease employment. 

Reinstatement of Coverage: If your eligibility ends and you return to work, you must work a minimum of 120 hours in one month and be actively working the first day of the next month to be eligible. (Example: work 120 hrs in January, reinstated effective February 1 st.)

               SELF-PAY TO MAINTAIN BENEFITS
If you are laid off on weekly accident & sickness, workers compensation, or leave of absence; you will be covered for one month following the last day worked, then you will be required to self-pay to maintain your health insurance coverage. The self-pay rates are for a maximum of 6 months, if you return to work in that time period, you will not have to go through the reinstatement process. If you do not return to work within the 6-month period, the rates will be increased to the COBRA rates, and you will have the remainder of the 18 months to pay for your benefits.

If you elect not to self–pay your benefits will terminate the month following the last date worked. When you return to work you will have to go through the REINSTATEMENT process.
