Self-Insured Dental Services

Vision Benefits
Vision benefits are covered by National Vision Administrators (NVA). You will receive a packet of information along with an ID card directly from NVA. Please use a participating doctor from the directory for the least out-of-pocket expense! Adults are eligible every 2-years; children under the age of 18 are eligible every year. Pension Plan
The “Shopmen’s Local Union No. 502 Pension Plan” (“the Plan” or the “Fund”)
was designed to help provide financial security for you and your family when
you retire. Social Security benefits, toward which you and your Employer
contribute equally, are in addition to the benefits provided by this Plan.
This booklet is called a Summary Plan Description (“SPD”) and is a summary
of the Pension Plan’s rules and benefits. It has been written in informal language for
your convenience and ease of understanding. It explains essential features of
the Plan and will be useful now and for your future reference. In all cases of
conflict, the text of the full, formal Plan Document will, of course, govern.
